Release of Liability for Participants (Read Before Signing)

IN CONSIDERATION OF the team and its players being allowed to participate in any way in the JLC Regional Committees, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

• The risk of injury to team participants from the activities involved in these programs is significant, including the potential for permanent disability and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
• I FOR MYSELF AND ON BEHALF OF THE TEAM AND ITS PLAYERS, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF OTHER PARTICIPANTS or organizers, and take full responsibility for the team’s participation.
• I willingly agree to comply with the program’s stated and customary terms and conditions. If I observe any concern in a team player’s readiness or in the program itself, I will withdraw the player(s) and bring it to the attention of the nearest official immediately.
• I, for myself, the team, and on behalf of all participants’ heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS the NC Hub committee; its directors, officers, officials, agents, employees, volunteers, sponsors, advertisers, and venue owners, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to the team’s participation, WHETHER ARISING FROM NEGLIGENCE OR OTHERWISE, to the fullest extent permitted by law.
• I HEREBY INDEMNIFY AND HOLD HARMLESS all the above from any and all liabilities incident to the team’s participation, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
• I understand that the tournament may involve compressed schedules due to holidays, and players may be required to play multiple games with limited rest between matches.
• I understand that risks may also arise from inclement weather, slipping, poor conditioning, inadequate equipment, or general hazards inherent to youth sports. Absolute safety cannot be guaranteed.

Waiver/Release for Communicable Diseases Including COVID-19

• Participation includes exposure to infectious diseases including but not limited to MRSA, influenza, and COVID-19. The risk of serious illness and death exists despite safety measures.
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF OTHERS, and accept full responsibility on behalf of the team.
• I agree to comply with all safety protocols regarding disease prevention. If I observe any unusual or significant hazard, I will remove the team member(s) and notify an official immediately.
• I, on behalf of the team and their families, HEREBY RELEASE AND HOLD HARMLESS the NC HUB committee, coaches, managers, volunteers, sponsors, and venue owners, WITH RESPECT TO ANY ILLNESS, DISABILITY, DEATH, or damages, WHETHER ARISING FROM NEGLIGENCE OR OTHERWISE, to the fullest extent allowed by law.
• All participants, parents, coaches, and volunteers are required to have medical insurance. The league is not responsible for any medical or other incidents occurring during the event.

Photo/Video Authorization

I hereby give my consent for the JLC Regional Committee to use photos/videos of the team and its players in brochures, websites, or promotional materials. I understand that these media assets remain the property of JLC and may be used for marketing purposes.

For Participants Under Age 18

I certify that, as the team representative with legal authority for players under the age of 18, I have communicated the provisions in this waiver to all parents/guardians and players. The participants and their guardians understand and accept the risks and responsibilities associated with participation. I do consent and agree to this release on behalf of the team, EVEN IF ARISING FROM NEGLIGENCE.
