Release of Liability for Participants (Read Before Signing)

IN CONSIDERATION OF being allowed to participate in any way in the JLC Regional Committees, related events, and activities as a coach or volunteer, the undersigned acknowledges, appreciates, and agrees that:

• The risk of injury from the activities involved is significant, including the potential for permanent disability and death. While rules and equipment may reduce the risk, serious injuries may still occur.
• I, FOR MYSELF, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF OTHERS, and assume full responsibility for my participation.
• I agree to comply with the program’s stated and customary rules and conditions. If I observe any concerning conditions, I will withdraw and inform an official immediately.
• I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS the NC Hub committee, its directors, officers, officials, employees, volunteers, sponsors, and property owners, FROM ANY AND ALL INJURY, DISABILITY, DEATH, or loss/damage to person or property, WHETHER ARISING FROM NEGLIGENCE OR OTHERWISE.
• I HEREBY INDEMNIFY AND HOLD HARMLESS all the above from any and all liability related to my participation, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

Waiver/Release for Communicable Diseases Including COVID-19

• Participation includes exposure to infectious diseases such as MRSA, influenza, and COVID-19. Despite precautions, risk of serious illness or death exists.
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, EVEN IF ARISING FROM NEGLIGENCE, and take full responsibility for my health and safety.
• I agree to follow all safety protocols related to infectious disease prevention. If I notice unsafe conditions, I will withdraw and notify the nearest official.
• I, for myself and on behalf of my heirs, HEREBY RELEASE AND HOLD HARMLESS the NC Hub committee and all associated parties from any illness, disability, death, or loss, EVEN IF ARISING FROM NEGLIGENCE.
• All participants including coaches must carry their own medical insurance. The league is not responsible for medical incidents during events.

Photo/Video Authorization

I give my consent for the JLC Regional Committee to use my photos or videos in any future brochures, flyers, websites, or promotional materials. I understand these remain the property of JLC and may be used for marketing or educational purposes.
