Release of Liability for Participants (Read Before Signing)

IN CONSIDERATION OF their child/athlete being allowed to participate in any way in the JLC Regional Committees, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

• The risk of injury to my child/athlete from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
• I FOR MYSELF, SPOUSE, AND CHILD/ATHLETE, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE PARTICIPANTS/PARENTS or others, and assume full responsibility for my child/athlete’s participation.
• I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/athlete’s readiness for participation and/or in the program itself, I will remove my child/athlete from participation and bring such to the attention of the nearest official immediately.
• I, for myself, my spouse, my child/athlete, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the NC Hub committee; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/athlete’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
• I HEREBY INDEMNIFY AND HOLD HARMLESS all the above from any and all liabilities incident to my child’s/athlete’s involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
• I understand that since the tournament is scheduled during the long weekend, game schedules will be compressed and kids are expected to play games with very little or no rest between consecutive games.
• I understand that certain risks, dangers, and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to youth sports programs exist. Therefore, it is recognized that it is impossible for the JLC Regional committee that hosts/organizes to guarantee absolute safety.

Waiver/Release for Communicable Diseases Including COVID-19

• Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE PARTICIPANTS or others, and assume full responsibility for my participation.
• I agree to comply with all stated and customary terms and conditions for participation as regards protection against infectious diseases. If I observe any unusual or significant hazard during my presence or participation, I will withdraw and inform the nearest official immediately.
• I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS “NC HUB committee”, their officers, coaches, team managers, volunteering parents, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“PARTICIPANTS”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
• All participants, parents, volunteers, coaches, and officials must have medical insurance. The league is not responsible for any medical or other incidents occurring during the event and participation.

Photo/Video Authorization

I hereby give my consent for the JLC Regional committee to use any photos/videos of myself and/or my minor child in future brochures, flyers, websites, photos, and videos of its programs and participants, for promotional and instructional purposes. I understand that all such photos/videos remain the property of the JLC Regional committee. I agree that pictures, videos, and scorecards associated with the NC HUB can be used for promotional and marketing purposes.

For Participants of Minority Age (Under Age 18 at the Time of Registration)

This is to certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/athlete, including the risks of participation and personal responsibility for adhering to safety and health protocols. Furthermore, my child/athlete understands and accepts these risks and responsibilities. I, for myself, my spouse, and child/athlete, do consent and agree to the release above, and I do release and agree to indemnify and hold harmless the Participants for any and all liabilities incident to my minor child’s/athlete’s participation in these activities, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
